The debate on limited health resource availability and fairness of distribution leads to economic arguments on scarcity of funds, and efficiency of financing. This paper focuses on the explanation of equity of resource distribution by focussing especially on financing health systems and how it affects equity in itself. It concentrates on an inter-country comparison emphasizing performance indicators, based on the WHR2000, and connecting them to health finance relevant numbers. Progressivity figures are used to highlight cross country equity differences. The analysis will provide evidence about the financing method mix' clusters and progressivity factor (Kakwani Index) influencing WHO DALE ranking. Regional and wealth perspectives are also looked at. Conclusively simple correlations can be observed for example between the Kakwani Index and the DALE ranks i.e. the more progressive a health care system is financed the better it is ranked. But also regarding financing mix' basic relationships can be found like social insurances leading to a better position contrary to the privately financed system mixes. The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

By 2007, health centers in primary health care are public entities, autonomous, contracted by HII, financed according to package of services, self managed. Health centre is managed by Manager and economist that are responsible for financial and human resource management. HC are financed performance based according to a new formula: 85% of budget was given monthly for normal functioning of services, 10% was given monthly as performance based and 5% was called quality bonus, given quarterly to HC. Study mean to measure impact of financial mechanism of health centers in function of health staff performance, by components: Finances, performance and level of health staff knowledge about reform. Target of study are primary health care centers, family doctors and nurse employed in selected health centers. 684 health staffs are interviewed by 52 health centers randomly selected as part of the survey. It is found positive statistical correlation between bonus level and health staff knowledge about reform(r=0.218, p=0.023), and continues medical education (r=0.607. p<0.001).
